残疾证被锁的话需要去残联解除。
残疾人证办理所需材料：
1、申请人身份证原件及其复印件、户口本原价及复印件。相关医疗机构的诊断病例（一年以上的恢复期）的原件及其复印件，身体本身存在明显的残疾可以不出具医疗机构的诊断及病历；
2、二寸白底近期彩色照片4张；
3、类别是精神、智力、未成年人户口不在一个户口簿的需要提供村级（社区）出具的监护人证明材料，监护人身份证、户口薄原件及其复印件。在医疗机构诊断或治疗过的人员，需提供诊断和病例；智力鉴定需出具村级（社区）的智力缺失的介绍信。
残疾人证办理流程：
1、申请人带齐上述“所需材料”到乡镇街残联领取并填写《中华人民共和国残疾人证申请表》；
2、乡镇街残联工作人员会一次性告知相关残联指定的评残鉴定医疗机构和具体鉴定时间；
3、指定医疗评残鉴定后，申请人带齐本人和监护人的身份证的原件及复印件、户口薄的原件及复印件、申请人鉴定后的《中华人民共和国残疾人申请表》及其近期彩色白底二寸照片，智力、精神、未成年人的监护人证明材料，送达所属地的公共服务中心录入。（监护人可以代办）；
4、县（市）政务大厅接收到各乡镇街上报评残人员的申请鉴定表、身份证复印件、户口薄复印件、一张白底近期彩色二寸照片，智力、精神、未成年人的监护人证明材料。审核合格后录入残疾人系统库，在十五个工作日内审核完成并打印制证。把证件邮寄到各乡镇街公共服务中心后发放到残疾人手中。

残疾人证坚持申领自愿、属地管理原则。凡符合残疾标准的视力、听力、言语、肢体、智力、精神及多重残疾人均可申领残疾人证。
