残疾证到期换证的办理流程为：如果残疾证到期的话，那么是可以凭借当事人旧的残疾证去当地残联换领新的残疾证，但是需要携带身份证、户口本和6寸免冠彩照到户籍所在地乡镇（街道）申请办残疾证，县残联对申请资料进行审核鉴定。

《中华人民共和国残疾人证管理办法》 第十九条 残疾人证有效期十年，期满可到批准残联免费换领，同时将原残疾人证交回。发证残联在新换领残疾人证的备注栏中注明换发信息，将回收的旧证统一销毁。
