释义	法律分析：(1)需要变更名义或者地址的，应当填写《变更商标申请人/注册人名义/地址申请书》，如果商标注册人或者申请人同时申请变更名义和地址，填写一份申请书即可;(2)需要变更商标代理人的，应当填写《变更商标代理人申请书》;(3)需要删减注册商标或注册申请中的商品或者服务项目的，应当填写《删减商品/服务项目申请书》，但不能对商品或服务项目加限定或修饰词句法律依据：《中华人民共和国商标法》四十一条注册商标需要变更注册人的名义、地址或者其他注册事项的，应当提出变更申请。
